The original Koneswaram temple, described in ancient Tamil literature as a magnificent thousand-pillared temple, was largely destroyed in 1624 by the Portuguese, who used the stone to build their own Fort Frederick on the same headland. The sacred swayambhu (self-manifested) lingam survived the destruction and was concealed in the sea by devotees; it was recovered in 1952 during dredging operations and returned to a rebuilt temple on the original site.

The current temple complex, rebuilt and restored through donations from Hindu communities worldwide, was consecrated in stages through the 20th and 21st centuries.
